This template is a starting point for you to design a screener for a more in-depth qualitative study. Ask demographic and behavioral questions to ensure a diverse and representative sample across characteristics such as age, gender, ethnicity, education level, and more.
(Please note that a good screener only asks for necessary demographic information. We’ve included a wide range for you to choose from, but try to limit yourself to the questions you really need to know.)
